Beneficiary Information Request Form – Effective June 15, 2020

Effective June 15, 2020, in compliance with new regulations in Illinois and California, the Beneficiary Information Request form (ORD 115116 2020) will be introduced. The company is required to collect certain information prior to a policy’s delivery on every policyowner, additional insured, and beneficiary listed on the application for life insurance to ensure that all benefits or proceeds are distributed to the appropriate persons upon the death of the insured.
